2027 GGNZ JAMBOREE YOUTH APPLICATION FORM

This application must be completed by the Parent/Guardian of the Youth Member applying.


Before you start this application:

  • Check the expiry date on your passport. it must not expire within 6 months of the return date of this trip. 

  • You will need two referees for this application - one Girl Guide member and one non-guiding person to provide a character reference for you. Contact them to check they are happy to fill out the online reference form generated once you complete this form.

  • It is your responsibility to follow up with your referees to ensure they complete their section of the form using the link emailed to them.  The process does not proceed to the next step until each person hits submit!

  • Please ensure that you tick the I am not a robot box when you finish you application, otherwise your form will not submit properly.


Permission to Travel

Youth Members travelling overseas with Girl Guides Australia require a Parent Consent to Travel Letter signed by ALL parents or guardians and witnessed by a Justice of the Peace or equivalent. Read more about children travelling without their parents.

If your application to represent Girl Guides Australia at the 2027 GGNZ Jamboree is successful, you will be sent the link to our consent form that must be completed and sent back to GGA office.

Please make sure they are aware of the trip details and will be available as an emergency contact for this Youth Member during that time.

Girl Guide & Outdoor Experience Reference

Provide contact details for a Girl Guide Leader/Adult in Guiding who knows the Youth Member and can fill out a short form as a recommendation for this trip. 

They will be asked to endorse this application based on the Youth Member's:

  • coping skills while on camps/overnights with Girl Guides

  • capacity to be away from home for an extended period

  • ability to adapt to changes.

Personal Reference

Please provide details for someone willing to provide a personal reference/recommendation for you. 

Please notify the person before submitting this application and ensure you have the correct email address. 

They will be sent an email with a link to a short form and should check junk mail for a snapforms email.

Travel Details

NOTE: If you do not currently hold a passport you can still apply to take part in the 2027 GGNZ Jamboree, however in order to attend the event you MUST have a valid passport prior to all travel being booked. The passport details must be of the passport the Youth Member intends to travel on.

Health and Wellbeing

The information provided in this section will be used by those responsible for the Youth Member's welfare and wellbeing (including medical treatment if required) at this event and access to this information will be restricted and disclosed accordingly.


Travelling overseas on new adventures and sharing accommodation is exciting but can also be stressful at times. This section provides information that can also be the key to a positive experience overseas.

It is very important that this information is honest and correct, because of the impact on:

  • the Youth Member's wellbeing

  • their ability to enjoy the trip

  • insurance cover

  • planning a successful trip for the whole Contingent.


Pre-existing medical conditions: If you do not declare pre-existing medical conditions, the GGA insurance policy could be invalid.  If your condition changes before the trip, you must tell the GGA Office.

Travellers with chronic(ongoing for 6 months or more) or complex conditions,must provide a GP care management plan or action plan.

A GP care management plan is a written plan of action that identifies your health and care needs, sets out services to be provided by your GP, and lists the actions you can take to help manage your condition.

This applies to all ongoing conditions such asthma, anaphylaxis, anxiety, adhd, diabetes, heart conditions etc; anything that you manage on an ongoing basis.

Speak to your doctor about including a plan for back up medication and medical devices, spare scripts, considerations for managing conditions while travelling and a plan for accessing support.

* Pre-existing medical conditions: If you become unwell on the trip due to a pre-existing medical condition, the GGA insurance policy could be invalid if you haven't shared all the key information about your health with GGA.
