Girl Guides Australia Good Service Awards Nomination Form

The purpose of the Adult Good Service Awards is to recognise and value the voluntary contribution of a Member to Girl Guiding. 

To recognise the contribution of a Member (the nominee), the Awards Committee needs to collect as much relevant information and examples as possible about their contribution.

There is information to assist you to complete this form in short explanations beneath some questions and in Guide Lines. 

When completing this form you can save and return to it at any time, you can make changes as necessary and leave blank any questions you are unable to complete.  Once the form is submitted you will receive an email with the completed  nomination form  attached as a PDF for your records.

Information about the Member you are nominating (the nominee)

Information about the nominee's current and past roles may be recorded at your Girl Guide State Office. The relevant information will be accessed by the Awards Committee for the purpose of assisting to determine which, if any award, is suitable for this nominee. This question is designed to capture any information you believe supports her award nomination but may not be held within her current State Girl Guide records.

The Awards Committee are seeking information about her years of involvement and any Trefoil Guild positions she may have held in her Guild, as part of the State Trefoil Guild or in a National Trefoil Guild role.
Trefoil Guild members do a great amount of community service but the Awards Committee are seeking information about service to Girl Guiding only. For example, she may have been an active member of a campsite committee or on a WAGGGS Friends Group committee.

Important Information

There may be a member who makes a contrubution that doesn't "fit neatly" into the assessment criteria framework. e.g. the Member may have volunteered on a short term project/committee at the national level but may not be involved with any activiites within her State. Alternatively, she may have been involved with a WAGGGS project team/committee for a period of time. 

The Awards system has flexibility to address these situations and Member contributions can be recognised. In these circumstances, the focus is more on the value and impact of the contribution and less on where the contribution was made.
